Fractional Laser for Acne Scars, Pores and Wrinkles: What Does It Really Treat?
Fractional laser is widely used to improve skin texture and is commonly considered for acne scars, enlarged pores, wrinkles and uneven skin.
However, these concerns are not all treated in exactly the same way.
"Fractional" describes how treatment is distributed across the skin rather than identifying one specific laser. Different fractional technologies, including fractional Erbium and fractional CO2, can produce different tissue effects, recovery periods and outcomes.
The condition being treated should therefore be diagnosed before the technology is selected.
How does fractional laser work?
Fractional treatment creates microscopic treatment zones while leaving areas of relatively untreated tissue between them.
A healing response follows, resulting in tissue renewal and collagen remodeling.
These processes can progressively improve skin texture, selected scars, fine lines and certain features of photoaging.
Does fractional laser treat acne scars?
Yes. Atrophic acne scarring is one of the important applications of fractional resurfacing.
Skin resurfacing and collagen remodeling can reduce the depth and visibility of selected scars and improve overall texture.
However, acne scars have different morphologies and do not all respond identically.
What types of acne scars are there?
Rolling scars typically appear as relatively broad depressions and can be associated with deeper fibrous tethering.
Boxcar scars have more defined edges and vary in depth.
Ice-pick scars are narrow at the surface and extend more deeply.
Identifying scar morphology is important because fractional laser may address certain components effectively while additional treatments may be required for others.
When is fractional laser alone not enough?
A tethered scar may require release of the underlying fibrous attachments.
Certain deep, narrow scars may require another targeted treatment.
Combination treatment can therefore be more appropriate than repeatedly resurfacing the skin without addressing the underlying scar anatomy.
Can fractional laser completely remove acne scars?
Complete scar removal should not be promised.
The objective is improvement in scar depth, visibility and surrounding skin texture.
The degree of improvement depends on scar morphology, depth, number of scars, laser technology, treatment parameters, number of sessions and individual response.
Does fractional laser treat enlarged pores?
Improvement in texture and collagen remodeling can make pores appear less prominent in selected patients.
However, "closing pores" is not medically accurate.
Pores are normal anatomical structures. Fractional laser does not permanently close or eliminate them.
The objective is to improve their visible appearance.
Why do pores look enlarged?
Pore appearance is influenced by sebaceous activity, genetics, loss of skin elasticity, aging and previous acne.
More than one contributing factor may therefore need to be addressed.
Can fractional laser treat wrinkles?
Fractional resurfacing can improve certain fine lines and wrinkles, particularly those related to skin aging, collagen changes and chronic photodamage.
Resurfacing and collagen remodeling can improve skin quality and reduce the appearance of selected lines.
Not every wrinkle, however, originates primarily within the skin.
What about dynamic wrinkles?
Dynamic wrinkles are largely created by repeated muscular contraction.
Fractional laser may improve the quality of the overlying skin but does not stop the muscular activity producing the wrinkle.
Dynamic wrinkles therefore require a different treatment strategy from lines primarily caused by cutaneous aging.
Can fractional laser treat sun damage?
Fractional resurfacing can form part of a treatment plan for features of photoaging such as rough texture and selected fine lines.
Pigmented lesions, however, require diagnosis because not all pigmentation disorders are the same.
Does fractional laser treat pigmentation?
Fractional laser is not a universal pigmentation treatment.
Certain pigmentary conditions may benefit from selected laser technologies or other approaches, while treatment-induced inflammation itself can trigger post-inflammatory hyperpigmentation in susceptible patients.
The pigmentation disorder should therefore be diagnosed before treatment.
What about fractional Erbium?
Fractional Erbium provides precise resurfacing and can be useful for selected textural irregularities, scars and fine lines.
Treatment intensity and depth should be selected according to the indication, skin characteristics and acceptable recovery period.
What about fractional CO2?
Fractional CO2 can provide greater thermal effects and more intensive resurfacing in certain settings.
Comparative acne-scar studies suggest that fractional CO2 may produce greater improvement in selected patients, but potentially with greater discomfort and recovery.
More intensive treatment is not automatically the best treatment for every patient.
Can fractional laser be used in darker skin types?
Fractional technologies can be used in selected patients across a range of skin types, but post-inflammatory hyperpigmentation becomes an important consideration.
Laser type, treatment intensity, previous pigmentation, sun exposure and post-treatment care should all be considered.
When do results appear?
Changes in skin texture may become visible after initial healing.
Collagen remodeling continues afterward, meaning improvement can develop progressively over the following weeks and months.
The final result should not be judged only a few days after treatment.
Will I need more than one session?
This depends on the indication, severity, laser technology and treatment intensity.
Certain patients may undergo a more intensive treatment, whereas other strategies use several less-intensive sessions.
There is no universal number appropriate for everyone.
Can fractional laser be combined with other treatments?
Yes.
This is particularly relevant for acne scars because several scar morphologies may coexist in the same patient.
Patients with facial aging may also have muscular wrinkles, volume loss or deeper laxity that require treatments other than resurfacing.
The bottom line
Fractional laser can be effective for improving atrophic acne scars, skin texture, the visible appearance of pores and certain wrinkles.
It does not treat every scar identically, permanently close pores, correct the muscular cause of dynamic wrinkles or serve as a universal treatment for pigmentation.
The best outcome begins with diagnosis.
Scar morphology, wrinkle type, causes of visible pores and skin characteristics should be assessed before selecting the fractional laser technology, treatment depth and parameters.
